'Afiyah is a wide word: safety, health, and being spared from trials. The Prophet ﷺ called asking for it one of the best things a person can ask for.

O Allah, keep my body well. O Allah, keep my hearing well. O Allah, keep my sight well. There is no god but You. O Allah, I seek Your protection from disbelief and from poverty. O Allah, I seek Your protection from the punishment of the grave. There is no god but You.

Source: Sunan Abi DawudRecite 3×, morning & evening

Abu Bakrah repeated these three times each morning and evening because he had heard the Prophet ﷺ pray with them.

    Allahumma inni as'alukal-'afwa wal-'afiyata fid-dunya wal-akhirah

O Allah, I ask You for pardon and for wellbeing in this world and the next.

Source: Sunan Ibn Majah · Sunan Abi Dawud

Asked which dua is best, the Prophet ﷺ answered three days running: ask your Lord for pardon and wellbeing in this world and the next (Sunan Ibn Majah). Sunan Abi Dawud reports a longer form he never left, morning or evening.

    Allahumma inni a'udhu bika minal-barasi wal-jununi wal-judhami wa min sayyi'il-asqam

O Allah, I seek Your protection from leprosy, madness, disfiguring disease, and every terrible illness.

Source: Sunan Abi Dawud · Sunan an-Nasa'i

    Allahumma inni a'udhu bika min jahdil-bala', wa darakish-shaqa', wa su'il-qada', wa shamatatil-a'da'

O Allah, I seek Your protection from trials too heavy to bear, from being overtaken by misery, from an evil outcome, and from enemies rejoicing at my misfortune.

Source: Sahih al-Bukhari · Sahih Muslim

The Prophet ﷺ used to seek refuge from these four things and told his companions to do the same; the opening “Allahumma inni a'udhu bika” is the usual way of putting that into words.
